ABSTRACT

RESUMO Objetivo Reunir os parâmetros encontrados no potencial miogênico evocado vestibular cervical (cVEMP) em crianças e adolescentes com síndrome do aqueduto vestibular alargado (SAVA) e identificar as possíveis alterações, quando comparados aos valores encontrados em normo-ouvintes da mesma faixa etária. Estratégia de pesquisa Revisão sistemática cadastrada na base PROSPERO, elaborada por meio de busca nos bancos de dados virtuais, a partir dos unitermos selecionados. Critérios de seleção Incluídos artigos científicos disponíveis na íntegra que relataram a avaliação com o uso do cVEMP na faixa etária entre 0 e 18 anos, com diagnóstico de SAVA, sem restrição de idioma e ano de publicação; excluídos estudos em paciente com algum distúrbio, outras patologias otoneurológicas e população fora da faixa etária estimada. Resultados Foram identificados 984 registros, a partir da pesquisa nas bases de dados consultadas e selecionados 5 artigos. Em um total de 133 pacientes que realizaram o cVEMP, foi observada presença de resposta na maioria dos casos, sem diferença significativa nas latências, mas com aumento na amplitude e diminuição nos limiares do cVEMP. Conclusão O teste cVEMP é recomendado na avaliação de crianças e adolescentes com SAVA e as características de aumento na amplitude e diminuição nos limiares podem ser utilizadas como parâmetros clínicos na identificação da referida síndrome, juntamente com a história clínica do paciente e os exames de imagem. No entanto, é imprescindível a realização de mais estudos com o exame cVEMP, ainda, em crianças e adolescentes com SAVA, para a melhor padronização dos valores encontrados, a fim de efetivar o diagnóstico correto.


ABSTRACT Purpose To gather the parameters found in the cervical vestibular evoked myogenic potential (cVEMP) in children and adolescents with enlarged vestibular aqueduct syndrome (SAVA) and identify the possible changes, when compared to the values found in normal hearing people of the same age group. Research strategy Systematic review registered in the PROSPERO database, prepared through a search in virtual databases, based on the selected keywords. Selection criteria Included scientific articles available in full that reported the evaluation using cVEMP in the 0 and 18 years old group , with a diagnosis of SAVA, without restrictions of language and year of publication; Studies on patients with any disorder other than otoneurological ones and populations outside the proposed age range were excluded. Results 984 records were identified from the search in the databases consulted and 5 articles were selected. In a total of 133 patients who underwent cVEMP, the presence of a response was observed in most cases, with no significant difference in latencies, but with an increase in amplitude and a decrease in cVEMP thresholds. Conclusion The cVEMP test is recommended in the evaluation of children and adolescents with SAVA and the characteristics of increase in amplitude and decrease in thresholds can be used as clinical parameters in the identification of this syndrome, together with the patient's clinical history and imaging exams. However, it is essential to carry out more studies with the cVEMP test, also in children and adolescents with SAVA, to better standardize the values found, in order to make the correct diagnosis.

Resumen Introducción: La cinetosis se relaciona con la presencia de una serie de síntomas que comúnmente son inducidos por situaciones cotidianas de viajes en medios de transporte. Una forma utilizada por décadas para determinar el grado de susceptibilidad a la cinetosis ha sido con la aplicación del cuestionario en su versión acortada Motion Sickness Suscep-tibility-short (MSSQ-short). Objetivo: Adaptar lingüística y transculturalmente al español el cuestionario MSSQ-short. Material y Método: Se llevaron a cabo cuatro etapas: Traducción directa, traducción inversa (retrotraducción), consolidación por un comité de expertos y pretest (aplicabilidad/viabilidad). En la etapa de pre-test 51 personas respondieron el cuestionario. Resultados: La discrepancias encontradas en las primeras etapas fueron resueltas por un tercer traductor, el cual concluyó en un documento final en español que fue analizado y revisado por el comité de expertos. Se determinaron los percentiles del 0 al 100, percentil 50 con 9,0 puntos, percentil 25 con 2,13 puntos y el percentil 75 con 17,4 puntos. La consistencia interna del cuestionario fue de 0,889. Conclusión: La traducción y adaptación transcultural fue aceptada por un comité de expertos y participantes con distintas características demográficas y educacionales. El cuestionario obtuvo buena consistencia interna y resultados concordantes con la versión original.


Abstract Introduction: Motion sickness is related to the presence of a series of symptoms that are typically induced by everyday situations of travel in means of transport. A way used for decades to determine the degree of susceptibility to motion sickness has been with the application of the questionnaire in its shortened version Motion Sickness Susceptibility-short (MSSQ-short). Aim: Linguistically and cross-culturally adapt the MSSQ-short questionnaire to Spanish. Material and Method: Four stages were carried out: direct translation, reverse translation (back translation), consolidation by a committee of experts, and pretest (applicability/feasibility). In the pre-test stage, 51 people answered the questionnaire. Results: The discrepancies found in the early stages were resolved by a third translator, which concluded in a final document in Spanish that was analyzed and reviewed by the expert committee. The percentiles from 0 to 100 were determined, 50th percentile with 9.0 points, 25th percentile with 2.13 points, and 75th percentile with 17.4 points. The internal consistency of the questionnaire was 0.889. Conclusion: The cross-cultural translation and adaptation were accepted by a committee of experts and participants with different demographic and educational characteristics. The questionnaire obtained good internal consistency and results consistent with the original version.

ABSTRACT

La pandemia por el virus SARS-CoV-2 causante de la enfermedad COVID-19 se ha expandido rápidamente a nivel mundial. Las autoridades de salud pública han recomendado limitar las prestaciones de salud no esenciales, buscando el equilibrio entre el riesgo del retraso en el tratamiento y la potencial exposición al virus. En la mayoría de las prestaciones en las áreas de audiología y otoneurología no es posible cumplir con el distanciamiento físico recomendado y la duración de la atención puede superar los 45 minutos, factores que aumentan el riesgo de contagio para el profesional. El objetivo de la presente revisión es describir la evidencia actual sobre las recomendaciones de atención para las áreas de audiología y otoneurología en contexto COVID-19. Además de la búsqueda de artículos científicos en diversas bases de datos, se revisaron las recomendaciones emitidas por las principales asociaciones en el área. La información obtenida se organizó considerando cuatro aspectos clínicos relevantes: prestaciones, procedimientos, elementos de protección personal y limpieza de equipamiento e insumos. La evidencia científica y las sugerencias de las asociaciones están en constante actualización y algunos temas se han desarrollado escasamente. Sin embargo, las recomendaciones coinciden en priorizar la atención a distancia, extremar el uso de elementos de protección personal, implementar protocolos de higiene de manos y limpieza de equipos e insumos. También, se sugiere incorporar estrategias de comunicación alternativas al lenguaje oral con las personas con hipoacusia para facilitar la discriminación de la palabra.


The COVID-19 pandemic caused by the SARS-CoV-2 virus has rapidly spread worldwide. Public health authorities have recommended limiting non-essential healthcare, in search of a balance between the risk of delaying treatment and the potential exposure to the virus. Most services in the audiology and otoneurologyareas can easily exceed 45 minutes and it is not possible to keep the recommended physical distancing. Both factors increase the risk of professionals being infected. Therefore, the purpose of this review is to describe the current evidence about recommendations in audiology and otoneurology care in the context of COVID-19. In addition to the search of scientific articles in various databases, recommendations issued by the mainassociations in the area were consulted. The information was gathered considering four relevant clinical aspects: health careservices, procedures, personal protective equipment and the cleaning of equipment and supplies. Scientific evidence and suggestions made by associations are being constantly updated, and therefore there is limited content on some topics. However, the recommendations agree on prioritizing remote health care, increasing the use of personal protective equipment, implementing protocols for hand hygiene and for the cleaning of equipment and supplies. The incorporation communication strategies other than oral language to interact with people with hearing lossto facilitate speech discrimination is also recommended.

ABSTRACT

Abstract Introduction: Neurotology is a rapidly expanding field of knowledge. The study of the vestibular system has advanced so much that even basic definitions, such as the meaning of vestibular symptoms, have only recently been standardized. Objective: To present a review of the main subjects of neurotology, including concepts, diagnosis and treatment of Neurotology, defining current scientific evidence to facilitate decision-making and to point out the most evidence-lacking areas to stimulate further new research. Methods: This text is the result of the I Brazilian Forum of Neurotology, which brought together the foremost Brazilian researchers in this area for a literature review. In all, there will be three review papers to be published. This first review will address definitions and therapies, the second one will address diagnostic tools, and the third will define the main diseases diagnoses. Each author performed a bibliographic search in the LILACS, SciELO, PubMed and MEDLINE databases on a given subject. The text was then submitted to the other Forum participants for a period of 30 days for analysis. A special chapter, on the definition of vestibular symptoms, was translated by an official translation service, and equally submitted to the other stages of the process. There was then a in-person meeting in which all the texts were orally presented, and there was a discussion among the participants to define a consensual text for each chapter. The consensual texts were then submitted to a final review by four professors of neurotology disciplines from three Brazilian universities and finally concluded. Based on the full text, available on the website of the Brazilian Association of Otorhinolaryngology and Cervical-Facial Surgery, this summary version was written as a review article. Result: The text presents the official translation into Portuguese of the definition of vestibular symptoms proposed by the Bárány Society and brings together the main scientific evidence for each of the main existing therapies for neurotological diseases. Conclusion: This text rationally grouped the main topics of knowledge regarding the definitions and therapies of Neurotology, allowing the reader a broad view of the approach of neurotological patients based on scientific evidence and national experience, which should assist them in clinical decision-making, and show the most evidence-lacking topics to stimulate further study.


Resumo Introdução: A otoneurologia é uma área de conhecimento que tem se expandido muito rapidamente. O estudo do sistema vestibular tem avançado tanto que mesmo definições básicas, como o significado dos sintomas vestibulares, foram apenas recentemente padronizadas. Objetivo: Apresentar uma revisão dos principais assuntos da otoneurologia, inclusive conceitos, diagnóstico e tratamento da otoneurologia, definir a evidência científica atual para facilitar a tomada de decisões e demonstrar as áreas mais carentes de evidência para estimular novas pesquisas. Método: Este texto é fruto do I Fórum Brasileiro de Otoneurologia, que reuniu os principais pesquisadores brasileiros dessa área para uma revisão da literatura. Serão feitos três trabalhos de revisão a serem publicados. Este primeiro abordou as definições e as terapias, o segundo abordará as ferramentas diagnósticas e o terceiro definirá os principais diagnósticos. Cada autor fez um levantamento bibliográfico na base de dados da Lilacs, SciELO, Pubmed e Medline de um determinado assunto. O seu texto foi então submetido aos demais participantes do Fórum por 30 dias para análise. Um capítulo especial, da definição dos sintomas vestibulares, foi traduzido por serviço de tradução oficial e igualmente submetido às demais etapas do processo. Houve então uma reunião presencial em que todos os textos foram apresentados oralmente e houve uma discussão entre os participantes para a definição de um texto consensual para cada capítulo. Os textos consensuais foram então submetidos a uma revisão final por quatro professores de otoneurologia de três universidades brasileiras e, por fim, finalizado. A partir do texto completo, publicado no site da Associação Brasileira de Otorrinolaringologia e Cirurgia Cérvico-Facial, foi escrita esta versão-resumo como artigo de revisão. Resultado: O texto apresenta a tradução oficial para o português da definição dos sintomas vestibulares propostos pela Barany Society e agrupa as principais evidências científicas para cada um das principais terapias existentes para as doenças otoneurológicas. Conclusão: Este texto agrupou de forma racional os principais tópicos de conhecimento a respeito das definições e terapias da otoneurologia, permite ao leitor uma visão ampla da abordagem dos pacientes otoneurológicos baseada em evidências científicas e experiência nacional, que deverá auxiliá-lo na tomada de decisões clínicas, e mostra os assuntos mais carentes de evidência para estimular novos estudos.

ABSTRACT

O novo coronavírus (SARS-Coronavírus-2: SARS-Cov-2) foi inicialmente relatado em 2019, em Wuhan, China, onde o aparecimento de muitos casos inexplicáveis de pneumonia severa deixou em alerta todos os serviços de saúde do mundo. A Organização Mundial da Saúde (OMS) declarou recentemente uma pandemia, com mais de um milhão de casos relatados até o momento de COVID-19. A relação da fisiopatologia deste novo agente infeccioso associado ao comportamento dos achados laboratoriais do exame do líquor-LCR, através das descrições de casos e relatos, é importante para a análise, interpretação e apoio diagnóstico.


The new coronavirus (SARS-Coronavirus-2: SARS-Cov-2), was initially reported in 2019 in Wuhan, China, where the appearance of many unexplained cases of severe pneumonia has put all health services on alert of the world. The World Health Organization (WHO) recently declared a pandemic, with more than 1,000,000 cases reported to date as of COVID-19. The pathophysiological relationship of this new infectious agent associated with the behavior of laboratory findings of Cerebrospinal Fluid (CSF) examination, through case descriptions and reports, are important for analysis, interpretation and diagnostic support.

ABSTRACT

RESUMO Objetivo Verificar a função dos canais semicirculares do labirinto de indivíduos com diabetes tipo 1, submetidos ao Video Head Impulse Test (v-HIT), e compará-los com indivíduos sem diabetes. Métodos Estudo transversal, observacional, analítico, realizado com uma amostra de conveniência, formada por 35 indivíduos diabéticos e 100 não diabéticos. Todos os participantes foram submetidos à avaliação vestibular por meio do v-HIT. Resultados A casuística foi composta por 135 participantes, divididos em dois grupos. O grupo de estudo foi composto por indivíduos com diabetes tipo 1, totalizando 21 mulheres e 14 homens. A idade variou entre 18 e 71 anos, com média de 35,37 anos e desvio padrão de 10,98. O grupo sem diabetes foi composto por 77 mulheres e 23 homens. A idade variou entre 20 e 83 anos, com média de 46,44 e desvio padrão de 19,82. Os grupos foram pareados entre si, com relação à idade (p=0,098) e sexo (p=0,052). Os pacientes diabéticos apresentaram ganho diminuído nos canais semicirculares posteriores e anterior esquerdo. A velocidade apresentou diferença significativa nos canais lateral esquerdo, anterior direito e posterior esquerdo no grupo com diabetes mellitus tipo 1, porém não apresentou correlação com o ganho dos canais semicirculares. Conclusão Os participantes com diabetes mellitus tipo 1 apresentaram um ganho diminuído nos canais semicirculares posteriores e no canal anterior esquerdo quando comparados com indivíduos não diabéticos.


ABSTRACT Purpose To verify the function of the labyrinth semicircular channels of type 1 diabetes individuals submitted to the Video Head Impulse Test (v-HIT) and to compare them with individuals without diabetes. Methods Cross-sectional, observational, analytical study conducted with a convenience sample of 35 diabetic and 100 non-diabetic individuals. All participants were submitted to vestibular evaluation using v-HIT. Results The sample consisted of 135 participants divided into two groups. The study group was composed of individuals with type 1 diabetes, totaling 21 women and 14 men. The age range was between 18 and 71 years, with a mean of 35.37 years and standard deviation of 10.98. The group without diabetes was composed of 77 women and 23 men. The age range was between 20 to 83 years, with a mean of 46.44 and standard deviation of 19.82. The groups were matched for age (p=0.098) and gender (p=0.052). Diabetic patients showed decreased gain in the posterior and left anterior semicircular canals. Velocity showed a significant difference in the left lateral, anterior right and posterior left canals in the group with DM1, however velocity did not show correlation with the gain of the semicircular canals. Conclusion participants with type 1 diabetes mellitus showed a decreased gain in the posterior semicircular canals and in the left anterior canal when compared to non-diabetic individuals.

ABSTRACT

INTRODUCCIÓN: Los trastornos del equilibrio constituyen un grupo heterogéneo de patologías. Son patologías frecuentes en medicina, representando una carga asistencial significativa en los distintos niveles de atención. En ese sentido, los estudios de frecuencia son una herramienta para entender patrones de enfermedad, como también la evaluación y planificación de una red asistencial. Material y método: Estudio descriptivo transversal sobre pacientes que asistieron al policlínico de otoneurología del Hospital Clínico Herminda Martin con motivo de consulta síntomas vestibulares durante el año 2018. Resultados: 183 pacientes cumplieron con los criterios de inclusión y exclusión, 78% (n=143) del sexo femenino y 22% (n=40) del masculino. Con una edad promedio de 58 años. El diagnóstico otoneurológico más frecuente fue vértigo posicional paroxístico benigno 34,97% (n=64), seguido de migraña vestibular 7,1% (n=13), enfermedad de Ménière 5,46% (n=10) y neuritis vestibular 4,37% (n=8). En 26,78% (n=49) no se logró un diagnóstico definitivo. Los trastornos de origen central representaron un 3,83% (n=7) y aquellos de origen no otoneurológico un 17,49% (n=32). Discusión: Los datos de frecuencia de los distintos trastornos del equilibrio registrados en el policlínico de otoneurología del Hospital Clínico Herminda Martin, son similares a la literatura internacional. Los trastornos vestibulares periféricos son los más frecuentes, de los cuales destaca el vértigo posicional paroxístico benigno representando el 34,97% del total de casos. Dentro de las causas otoneurológicas, la migraña vestibular fue la segunda causa en nuestro medio con un 7,1% del total de casos.


INTRODUCTION: Balance disorders are a heterogeneous group of diseases. They are a frequent medical condition, representing a significant healthcare burden at the different levels of care. Thus, frequency studies are a tool to understand disease patterns, as well as the evaluation and planning of a healthcare network. Material and method: Cross-sectional study of patients who attended the otoneurology out-patient clinic of the Hospital Clinico Herminda Martin with chief complaint of vestibular symptoms during 2018. Results: 183 patients met the inclusion and exclusion criteria, 143 (78%) were female and 40 (22%) male. With an average age of 58 years. The most frequent otoneurological diagnosis was benign paroxysmal positional vertigo 34.97% (n=64), followed by vestibular migraine 7.1% (n=13), Ménière's disease 5.46% (n=10) and vestibular neuritis 4.37% (n=8). In 26.78% (n=49) a definitive diagnosis was not achieved. Disorders of central origin represented 3.83% (n=7) and non-otoneurological origin 17.49% (n=32). Discussion: The frequency data of the balance disorders registered in the otoneurology out-patient clinic are similar to the international literature, peripheral vestibular disorders are the most frequent, of which benign paroxysmal positional vertigo stands out, representing 34.97% of the total cases. Among the otoneurological causes, vestibular migraine was the second cause in our study with 7.1% of the total cases

RESUMEN Los sistemas vestibular, visual y somatosensorial coordinan el control postural y se adaptan a las actividades cotidianas en diferentes contextos, sin embargo, cuando existe un cuadro clínico con sintomatología vestibular, las ponderaciones de estos sistemas cambian mayoritariamente al sistema visual, creando una dependencia visual. El cerebelo, que es el principal encargado de generar una compensación vestibular en casos de hipofunción vestibular, juega un rol destacado en el aprendizaje motor generando comportamientos adaptativos que se transforman en puntos claves en la rehabilitación vestibular. Este artículo pretende exponer lo que la literatura informa en relación a la dependencia visual en pacientes con hipofunción vestibular.


ABSTRACT The vestibular, visual and somatosensory systems coordinate postural control and adapt to daily activities in different contexts, however, when there is a clinical picture with vestibular symptoms, the weights of these systems change mainly to the visual system, creating a visual dependence. The cerebellum, which is the main responsible for generating vestibular compensation in cases of vestibular hypofunction, plays a prominent role in motor learning generating adaptive behaviors that become key points in vestibular rehabilitation. This article aims to expose what the literature reports in relation to visual dependence in patients with vestibular hypofunction.

ABSTRACT The main objective of this manuscript is to describe some personal and professional aspects of two outstanding researchers and pioneers in neuro-otology. Among other achievements, Margaret Ruth Dix, and Charles Skinner Hallpike, designed the most-used provocative test for positional nystagmus for the diagnosis of lithiasis in the vestibular posterior semicircular canal.


RESUMO O principal objetivo desse manuscrito diz respeito a alguns aspectos pessoais e profissionais de dois excelentes pesquisadores e pioneiros da neuro-otologia. Entre outras descrições, Margaret Ruth Dix e Charles Skinner Hallpike descreveram o teste provocativo mais utilizado de nistagmo posicional para o diagnóstico de litíase no canal semicircular posterior do sistema vestibular.

ABSTRACT

RESUMO Estudo retrospectivo, transversal e descritivo, aprovado pelo Comitê de Ética em Pesquisa (CEP), sob o número CAAE 16728013.0.0000.5346. Trata-se de uma série de casos que tem por objetivo investigar a presença de sintomas otoneurológicos e o equilíbrio postural de seis pacientes com tontura após traumatismo cranioencefálico (TCE). Foram submetidos a uma anamnese clínica e a avaliações: teste de organização sensorial por meio da posturografia dinâmica foam laser e provas oculomotoras e vestibulares da vectoeletronistagmografia computadorizada (VENG). Os dados foram analisados a partir dos valores de referência para cada avaliação. Na posturografia, observou-se que as maiores diferenças entre o valor obtido e o valor de referência foram nas posições VI, V e III, respectivamente. A análise sensorial indicou alteração, principalmente, nos sistemas de preferência visual e vestibular. Todos os pacientes avaliados apresentaram ao menos uma alteração nas provas da VENG. Cinco pacientes tiveram alteração na prova calórica e nenhum apresentou alteração na prova rotatória pendular decrescente, que avalia a compensação vestibular. Além das queixas vestibulares, quatro pacientes estavam em tratamento com psicotrópicos para depressão. Tais resultados ratificam a presença de alterações vestíbulo-oculares no pós-TCE, os quais devem receber atenção especial devido ao comprometimento central associado.


ABSTRACT It is a retrospective, cross-sectional, descriptive, approved by Ethics Research Committee (ERC), under number CAAE 16728013.0.0000.5346. This is a series of cases that aims to investigate the presence of otoneurological symptoms and the postural balance of six patients with dizziness after Traumatic Brain Injury (TBI). Participants were submitted to a clinical anamnesis and the evaluations: Sensory Organization Test through Foam Laser Dynamic Posturography and oculomotor and vestibular tests of Computerized Vectoelectronystamography (VENG). The data were analyzed from the reference values for each evaluation. In posturography, it was observed that the greatest differences between the value obtained and the reference value were in positions VI, V and III, respectively. Sensory analysis indicated alteration mainly in the visual and vestibular preference systems. All the evaluated patients presented at least one alteration in the VENG tests. Five patients had alterations in the caloric test, and none presented alterations to the rotatory chair test (RCT), which evaluates the vestibular compensation. Considering vestibular complaints, four patients were on psychotropic treatment for depression. These results demonstrate the presence of vestibulo-ocular alterations in post-TBI, which should receive special attention due to associated central impairment.

Objetivo: los trastornos del equilibrio en la edad pediátrica muestran un desafío diagnóstico y terapéutico para pediatras y otorrinolaringólogos, dada la dificultad para la descripción de síntomas. En Latinoamérica son pocos los estudios sobre el perfil epidemiológico de los pacientes pediátricos valorados por trastornos del equilibrio. El presente trabajo pretende describir las tendencias en la presentación de patología vestibular en pacientes pediátricos valorados en los últimos tres años en el Hospital Británico de Buenos Aires. Metodología: estudio observacional, descriptivo, retrospectivo. Se revisaron las historias clínicas de menores de 16 años, quienes consultan por trastornos del equilibrio en el periodo 2013-2016. Resultados: el estudio mostró una media etaria de 9.07 años y se observa mayor número de consultas por pacientes del sexo femenino. Los principales síntomas de consulta fueron vértigo y mareo en mujeres y mareo en hombres. El principal diagnóstico en ambos sexos fue migraña vestibular. Tanto en hombres como en mujeres que tuvieron por síntoma de consulta vértigo, los diagnósticos finales fueron vértigo posicional paroxístico de la infancia y migraña vestibular. Conclusiones: en la población estudiada el síntoma que generó más consultas fue vértigo. Las consultas fueron en mayor número de niñas con una media de edad de 9.07 años para toda la población estudiada. El diagnóstico más frecuente tanto en niñas como en niños fue migraña vestibular.


Objective: Balance disorders in patients during pediatric ages are a diagnostic and therapeutic challenge for pediatricians and otorhinolaryngologists due to the difficulty of symptom description. In Latin America, there are few studies on the epidemiological profile of pediatric patients with balance disorders. The aim of this study is to describe the trends in vestibular pathology in pediatric patients seen over the last three years at Hospital Británico de Buenos Aires. Methodology: Observational, descriptive, retrospective study. We reviewed the medical history of patients under 16 who sought medical attention due to balance disorders from 2013 to 2016. Results: Study participants had a mean age of 9.07, of whom more patients were female. The main symptoms were vertigo and dizziness in girls and dizziness in boys. The main diagnosis for both girls and boys was vestibular migraine. For patients (both boys and girls) whose main symptom was vertigo, final diagnosis was infant paroxysmal positional vertigo and vestibular migraine. Conclusions: In the population studied, the symptom most commonly leading to seeking medical attention was vertigo. More girls sought medical attention than boys, and the mean age of the total population studied was 9.07. The most frequent diagnosis, both in girls and boys, was vestibular migraine.

Na América Latina são poucos os estudos sobre o perfil epidemiológico dos pacientes pediátricos valorados por transtornos do equilíbrio. O presente trabalho pretende descrever as tendências na apresentação de patologia vestibular nos pacientes pediátricos valorados nos últimos três anos no Hospital Britânico de Buenos Aires. Metodologia: estudo observacional, descritivo, retrospectivo. Se revisaram as histórias clínicas de menores de 16 anos, quem consultam por transtornos do equilíbrio no período 2013-2016. Resultados: o estudo mostrou uma média etária de 9.07 anos e se observa maior número de consultas por pacientes do sexo feminino. Os principais sintomas de consulta foram tontura e enjoo em mulheres e enjoo nos homens. O principal diagnóstico em ambos sexos foi a enxaqueca vestibular. Tanto nos homens quanto nas mulheres que tiveram por sintoma de consulta tontura, os diagnósticos finais foram tontura posicional paroxístico da infância e enxaqueca vestibular. Conclusões: na população estudada o sintoma que gerou mais consultas foi tontura. As consultas foram em maior número de meninas com uma idade média de 9.07 anos para toda a população estudada. O diagnóstico mais frequente tanto em meninas quanto em meninos foi enxaqueca vestibular.

RESUMO Introdução O Potencial Evocado Miogênico Vestibular Cervical (cVEMP) tem se tornado uma medida fidedigna e complementar da avaliação vestibular. A investigação das alterações de ordem vestibular na população pediátrica é de grande importância, pois essas alterações podem acarretar uma série de repercussões ao longo do desenvolvimento. Objetivo: Avaliar e analisar as respostas do cVEMP em crianças e adolescentes e comparar esses achados entre gênero, idade e orelhas. Métodos Estudo transversal, constituído por 35 crianças e adolescentes, 18 do gênero feminino e 17 do masculino, de 7 a 18 anos de idade, possuindo limiares auditivos normais e sem queixas otoneurológicas. Todos realizaram avaliação auditiva periférica e cVEMP. Resultados Na análise, observou-se que a média das latências das ondas P1 e N1 foi, respectivamente, 15,92 ms e 24,32 ms, da amplitude de P1/N1 foi de 36.91 µv e a razão de simetria apresentou média de 22,95%. Não foram encontradas diferenças estatisticamente significativas na comparação de gênero e orelhas. O mesmo ocorreu no índice de assimetria que foi comparado entre os gêneros. Na comparação entre grupos de idade, também não foram evidenciadas diferenças estatisticamente significativas nas latências e amplitudes. Conclusão Foi possível mensurar valores para latência e amplitude das ondas P1 e N1, em crianças e adolescentes. Não houve diferença nas comparações entre as orelhas, gênero e faixa etária.


ABSTRACT Introduction The Cervical Vestibular Myogenic Evoked Potential (cVEMP) has become a reliable and complementary measure of vestibular assessment. The investigation of vestibular disorders in the pediatric population is of great importance because they can have a series of repercussions throughout development. Purpose To evaluate and analyze cVEMP responses in children and adolescents and compare them among gender, age, and ears. Methods A cross-sectional study with 35 children and adolescents, 18 females and 17 males, aged 7 to 18 years, with normal hearing thresholds and no otoneurological complaints. All of them underwent a peripheral auditory evaluation and cVEMP. Results In the analysis, it was observed that the mean latencies of P1 and N1 waves were, respectively, 15.92 ms and 24.32 ms, the amplitude P1/N1 of 36.91 µv and the symmetry ratio presented an average of 22.95%. No statistically significant differences were found comparing gender and ears. The same happened in the asymmetry index between genders. There were also no statistically significant differences in latencies and amplitudes. Conclusion It was possible to measure values for latency and the amplitude of P1 and N1 waves in children and adolescents. In the present study, there was no statistically significant difference in the comparisons among ears, gender, and age.

ABSTRACT

Neuro-otologic symptoms such as dizziness, hearing loss, or tinnitus give rise to peripheral change-induced neuroplasticity or central pathology-induced structural or functional changes. In this regard, functional neuroimaging modalities such as positron emission tomography (PET), functional magnetic resonance imaging (fMRI), magnetoencephalography (MEG), quantitative electroencephalography (qEEG), or functional near infrared spectroscopy have provided researchers with possibility to observe neuro-otologic disease-induced central functional changes. Among these methods, PET and fMRI are advantageous over qEEG or MEG with regard to spatial resolution, while qEEG and MEG are advantageous over PET or fMRI with regard to temporal resolution. Also, fMRI or MEG is not suitable for patients with implanted devices, whereas PET is not ideal for repetitive measures due to radiation hazard. In other words, as these modalities are complementary to one another, researchers should choose optimum imaging modality on a case by case basis. Hereinafter, representative functional neuroimaging modalities and their application to neuro-otologic research will be summarized.

ABSTRACT

Objetivo Avaliar o equilíbrio corporal de pacientes portadores de disfunção vestibular, por meio da posturografia dinâmica computadorizada. Métodos Estudo com 116 indivíduos com disfunção vestibular periférica, com idades entre 22 anos e 6 meses e 94 anos e 1 mês. Foram submetidos à avaliação otoneurológica completa, avaliação otorrinolaringológica, além da posturografia dinâmica computadorizada com os testes de organização sensorial, controle motor e de adaptação. Resultados O teste de organização sensorial foi o mais sensível para detectar alterações do equilíbrio e a condição 4, em que os pacientes permanecem na posição ortostática, com os pés afastados e sobre os sensores da superfície de referência, com os olhos abertos; condição 5, na qual os pacientes continuam na posição com os olhos fechados e a condição 6, em que os pacientes mantém os olhos abertos e o campo visual sofre deslocamentos anteroposteriores. Nas três condições, a superfície de referência oscila com movimentos similares à gangorra e foram as que os participantes apresentaram maiores dificuldades. Houve maior ocorrência de quedas se compararmos ao risco de quedas, avaliado quando o paciente tem pontuação entre 60 e 69 no índice de equilíbrio, principalmente a partir da condição 4 do teste de organização sensorial e entre os idosos. As mulheres tiveram maior número de quedas durante os testes. Conclusão A posturografia dinâmica computadorizada detectou as alterações relacionadas ao equilíbrio corporal, auxiliando no diagnóstico das disfunções e complementando a avaliação vestibular. .


Purpose To evaluate the body balance of patients with vestibular dysfunction with computerized dynamic posturography. Methods Study of 116 individuals with peripheral vestibular dysfunction, aged 22 years and 6 months old to 94 years and 1 month old were subjected to a complete otoneurological assessment; computerized dynamic posturography with a sensory organization test, motor control test, and adaptation test; and an otolaryngological evaluation. Results The sensory organization test detected balance changes with the most sensitivity. Condition 4, during which the patients stood with their feet apart with them placed on the sensors of the reference surface and with their eyes open; condition 5, during which the patients remained in position with their eyes closed; condition 6, during which the patients kept their eyes open and their visual field underwent anterior-posterior displacement; and all three conditions when the reference surface moved in a see-saw manner were the conditions during which the participants had the greatest difficulties. The results showed patients with composite scores between 60 and 69, which indicated a higher occurrence of falls compared to the risk of falls, during condition 4 of the sensory organization test and among the elderly. Women had a higher number of falls during testing. Conclusion Computerized dynamic posturography, which concisely detected body balance changes, can be used in the diagnosis of vestibular disorders to complement vestibular assessments. .

ABSTRACT

Patients with recurrent vertigo/dizziness or unsteadiness are a heterogeneous group of complex disorders affecting the peripheral and central vestibular system. They represent a diagnostic challenge for the clinicians, and their genetic basis is largely not known. However, there are some cerebellar and vestibular disorders with a strong genetic background, such as episodic ataxia, spinocerebellar ataxia, vestibular migraine, Meniere's disease, and autosomal dominant nonsyndromic deafness. Furthermore, recent advances in next generation sequencing technique are increasing the number of novel genes associated with cerebellar and vestibular disorders. In this article, we have summarized clinical and molecular genetics findings in neuro-otology.

ABSTRACT

This paper presents diagnostic criteria for Meniere's disease jointly formulated by the Classification Committee of the Barany Society, The Japan Society for Equilibrium Research, the European Academy of Otology and Neurotology, the Equilibrium Committee of the American Academy of Otolaryngology-Head and Neck Surgery, and the Korean Balance Society. The classification includes two categories: definite Meniere's disease and probable Meniere's disease. The diagnosis of definite Meniere's disease is based on clinical criteria and requires the observation of an episodic vertigo syndrome associated with low- to medium- frequency sensorineural hearing loss and fluctuating aural symptoms (hearing, tinnitus and/or fullness) in the affected ear. Duration of vertigo episodes is limited to a period between 20 minutes and 12 hours. Probable Meniere's disease is a broader concept defined by episodic vestibular symptoms (vertigo or dizziness) associated with fluctuating aural symptoms occurring in a period from 20 minutes to 24 hours.
